Our palliative care program focuses on improving the quality of life of patients with serious or life threating diseases, i.e Cancer-HIV, end of life . It is care provided by nurses & PSW's within the clients homes or in health care facilities. our goal is to provide our patients relief from pain & other symptoms of a serious illness no matter the diagnosis or the state of disease.
Our team of Nurses & PSW's will collaborate with the family and other professionals in direct care with the our patients to create an individualized care plan.
